          Accident profile

         At 15:30 on August 10, 2023, the first vehicle to replace the limit device in the west of Gate 9 of the steel pipe harness workshop of Zhejiang ** Steel Structure Manufacturing Co., LTD., overturned due to the support leg not opening when the electric walking shear fork aerial work platform was lifted for the second time (about 7 meters), resulting in one death.         Accident course

        At 14:25 p.m. on August 10, 2023, Hu **, an electrician from the machine repair class of the production Department, received the fault repair of the first vehicle replacement limit device on the west side of Gate 9 of the steel pipe bundle workshop.

       At 14:35, electrician Hu ** opened the electric walking shear fork aerial work platform (hereinafter referred to as the lifting work platform) to the middle ground of the 9th gate, and called the guardian Wu ** of the shift.           At about 15:30, Hu ** climbed on the lifting operation platform again without opening the support leg and started to raise. When he rose to a height of about 7 meters, the lifting operation platform suddenly overturned to the middle of the north steel pile, causing Hu ** to fall from the height and fall head down to the gap ground in the steel pile.

         According to the investigation at the scene of the incident, the incident place is the first driving place west of Gate 9 of steel pipe bundle workshop of Zhejiang ** Steel Structure Manufacturing Co., LTD. The lifting operation platform has turned over to the north, the platform car has been spread out about 7 meters, and the ground has obvious scratches. The support leg of the lifting platform is not open. There's obvious blood on the floor at the north steel stack gap. There's a safety rope on the platform, and the platform guardrail is out of shape.

        Direct cause of accident

        The accident investigation team through the accident scene investigation, technical appraisal, access to monitoring and questioning personnel and other means, after experts' demonstration, comprehensive analysis determined that the direct cause of the accident is: Electrician Hu ** weak safety awareness, in violation of the "Company climbing vehicle operating regulations", habitual illegal risky operations, did not open the support leg of the shear fork aerial work platform lifting operation, resulting in instability and rolling is the direct cause of the accident.
